Where in the World Is Baldness Most Common?
Does Geography, Ancestry—or Simply Getting Older—Determine Who Loses Their Hair?
Look around almost any sufficiently large group of adult men and sooner or later you'll find one.
A receding hairline.
A thinning crown.
Perhaps the familiar horseshoe of hair remaining around the sides and back of the scalp.
Male-pattern baldness occurs throughout the world.
But does it occur equally throughout the world?
Are men in Europe more likely to become bald than men in Asia?
What about the Middle East?
Africa?
The Pacific?
And if baldness is influenced so strongly by genetics, might there actually be populations in which losing one's hair is comparatively unusual?
The answer is:
Yes—there are real population differences.
But determining which country is “the baldest country in the world” is considerably harder than many internet rankings would have us believe.
And there's another complication.
Perhaps the single strongest predictor of whether someone will exhibit pattern hair loss isn't where that person lives at all.
It's how old that person is.
First, What Kind of Baldness Are We Talking About?
“Baldness” is a convenient everyday word, but it can describe several very different conditions.
Hair can be lost because of:
- autoimmune disease;
- illness;
- medications;
- nutritional deficiencies;
- scarring disorders;
- physical traction;
- hormonal changes;
- physiological stress;
- and numerous other causes.
For this discussion, we're primarily talking about androgenetic alopecia—the familiar progressive pattern hair loss seen in men and women.
In men, it commonly produces recession at the temples, thinning of the crown, and eventually substantial loss across the top of the scalp.
In women, pattern hair loss more commonly appears as progressive thinning and reduced density, particularly through the central scalp, rather than the completely bare crown often associated with advanced male-pattern baldness.
Androgenetic alopecia is strongly influenced by:
genetics + androgen biology + follicular susceptibility + age.
Those variables aren't distributed identically among every population.
And that's where our journey around the world begins.
So Where Is Baldness Most Common?
The medical literature generally reports that male androgenetic alopecia is particularly common among populations of European ancestry.
Major clinical reviews report both earlier onset and greater severity among European/Caucasian populations compared with several Asian populations.
That doesn't mean:
Europeans go bald and Asians don't.
Far from it.
Pattern hair loss occurs throughout Asia.
But population studies from China, Korea, Taiwan and Japan have generally reported lower prevalence, later onset, or both compared with European-derived populations.
That's an important distinction.
A population can have a lower rate of baldness at age 40 while still having a substantial rate by age 70.
What About China?
China gives us some particularly good data because researchers conducted a large community-based study across six cities involving more than 15,000 completed participants.
Among men, the overall prevalence of androgenetic alopecia was about:
21.3%
But look what happened when the researchers separated the men by age:

| 50–59 | 31.9% |
| 60–69 | 36.2% |
| 70+ | 41.4% |
Among Chinese women in the same study, overall prevalence was approximately 6%, also rising progressively with age.
That table teaches us something enormously important.
The question:
“What percentage of Chinese men are bald?”
isn't nearly as useful as:
“What percentage of Chinese men of a particular age show androgenetic alopecia?”
Age completely changes the answer.
What About Korea and Japan?
A Korean study cited in Asian clinical guidelines found androgenetic alopecia of Norwood grade III or greater in about 14.1% of men overall, again increasing substantially with age.
Japanese men present another interesting pattern.
Research summarized in major clinical reviews suggests that male-pattern hair loss in Japanese men tends to begin approximately a decade later than in European men. Asian consensus guidelines have also reported lower prevalence within comparable decades of life.
That's fascinating because the difference isn't simply:
bald versus not bald.
It can also be:
When does the process begin?
A population in which significant thinning commonly begins at 45 will look much less bald in a cross-sectional survey than one in which it commonly begins at 30—even if many men in both populations eventually develop hair loss.
Thailand Complicates the Picture
And this is exactly why we shouldn't make sweeping statements about “Asian baldness.”
A study of 1,124 Thai and Chinese men living in Bangkok found cosmetically significant male-pattern baldness in approximately 38.5% of participants.
Prevalence increased steadily with age and approached rates reported in European populations among older subjects.
So:
Asia isn't one population.
Neither is Europe.
Neither is Africa.
Neither is America.
The genetic landscape doesn't respect the convenient boundaries on our maps.
What About Africa?
Here the evidence becomes thinner—and that itself is important.
Some clinical literature reports lower prevalence and/or less extensive pattern hair loss among Black men than among men of European ancestry. One recent clinical review describes male androgenetic alopecia as substantially more frequent in European-derived populations than in Black American populations.
But we should be cautious.
Historically, dermatologic research has not sampled every ancestral population equally well.
And hair loss among Black populations introduces additional considerations because other forms of alopecia—such as traction alopecia and central centrifugal cicatricial alopecia—can also be important.
So I would not publish a neat percentage claiming:
“X% of African men go bald.”
We don't have sufficiently comparable population-wide data to make that statement responsibly.
Sometimes the scientifically correct answer really is:
We need better data.
What About Native American and Indigenous Populations?
Several clinical references describe conventional male-pattern baldness as comparatively less prevalent among Native American and Inuit populations than among European-derived populations.
Again, however, the available epidemiology is much less extensive than it is for European and East Asian populations.
So these findings are interesting, but I wouldn't turn them into a world championship table.
Which brings us to...
Who Wins the Baldness World Cup?

Search the internet and you'll find wonderfully confident rankings:
THE 10 BALDEST COUNTRIES IN THE WORLD!
Spain!
Italy!
France!
The United States!
Germany!
The Czech Republic!
Pick a survey and somebody wins.
The problem is that comparing baldness among countries requires comparable:
- age distributions;
- diagnostic definitions;
- sampling methods;
- sex ratios;
- ancestry distributions;
- classification systems;
- and severity thresholds.
Without those controls, country rankings can become surprisingly misleading.
Imagine Country A has a median age of 47.
Country B has a median age of 27.
Even if both populations had identical genetic susceptibility, Country A would probably appear considerably balder simply because androgenetic alopecia becomes much more common with age.
That's not a baldness gene.
That's arithmetic.
Nationality Isn't Ancestry
There's another obvious problem.
What exactly is an “American” genetically?
Someone whose ancestors came from Ireland?
China?
Nigeria?
Mexico?
Japan?
Norway?
India?
All of the above?
Modern countries contain genetically diverse populations.
A person of Japanese ancestry living in California doesn't suddenly acquire the follicular genetics of someone whose ancestors came from Scotland.
Likewise, moving a Scottish man to Tokyo doesn't persuade his androgen receptors that he's Japanese.

So although geographic studies are useful, the biologically interesting variables involve ancestry, genetics and population history, not the passport in someone's pocket.
What About the Middle East?
This is where your original intuition was interesting.
It would be easy to look at Mediterranean and Middle Eastern populations and assume that male-pattern baldness must be especially common there.
But the scientific literature doesn't support a simple:
Middle East = world capital of baldness
conclusion.
There are individual studies of Middle Eastern and Mediterranean populations, but the data aren't sufficiently standardized across countries and ages to establish a reliable global hierarchy.
Some populations may indeed show substantial prevalence.
But again:
ancestry + age + diagnostic criteria + sampling
matter enormously.
So I wouldn't tell a True Tape reader that Turkey, Israel, Iran, Lebanon, Greece or another particular country is unusually bald unless we had comparable age-adjusted epidemiology to prove it.
America Gives Us a Useful Benchmark
One classic U.S. study cited in clinical reviews found moderate-to-severe male androgenetic alopecia in approximately 53% of men aged 40–49.
Notice what we're not saying:
“53% of American men are bald.”
We're saying that within one studied age group and using a particular severity definition, slightly more than half showed moderate-to-severe male-pattern hair loss.
Other medical reviews commonly summarize the broad age relationship among European-derived men as roughly:
30% around age 30
50% around age 50
with prevalence continuing upward with advancing age.
Those aren't laws of nature.
They're useful population approximations.
But they demonstrate just how common androgenetic alopecia really is.
By Old Age, Keeping All Your Hair Is Increasingly the Exception
Some clinical references estimate that approximately 80% of European/Caucasian men show some degree of androgenetic alopecia by age 70.
That puts the cultural perception of baldness in an interesting light.
We often treat male-pattern hair loss as though something unusual has happened.
Biologically, among older men of some ancestries, it may be considerably more unusual to have no evidence of pattern hair loss at all.
The unusual thing isn't necessarily that Grandpa lost his hair.
It may be that Grandpa Harry reached 78 looking like a shampoo model.
Women Are Very Much Part of This Story
Baldness is often discussed as though it were exclusively male.
It isn't.
Female-pattern hair loss is common and also increases dramatically with age.
One U.S. study of approximately 1,000 women of European ancestry reported an overall prevalence around 19%, increasing from approximately 3% among women in their twenties to nearly 30% among women in their seventies and eighties.
Other populations and studies produce different estimates. A recent clinical review summarizes evidence suggesting that signs of female-pattern hair loss may be present in roughly 41–50% of women aged 70 and older, depending upon diagnostic criteria and population.
And population differences appear among women too.
Studies summarized in the literature have found lower overall prevalence among Korean and Chinese women than among several European-derived study populations.
Again:
Age matters enormously.
Why Does Female Hair Loss Seem Less Common?
Partly because it often looks different.
Advanced male androgenetic alopecia can produce an unmistakable bare scalp.
Female-pattern hair loss more commonly produces:
widening of the part
reduced density
central scalp thinning
while often preserving the frontal hairline.
A woman may therefore have substantial follicular miniaturization without appearing “bald” in the stereotypical sense.
That means everyday observations probably underestimate the amount of pattern hair loss occurring among women.
So Why Do Populations Differ?
The most defensible answer is:
Genetics.
Androgenetic alopecia is polygenic—many genetic variants contribute to susceptibility.
Those variants don't occur at identical frequencies in every ancestral population.
We already encountered this in our article:

There isn't one universal baldness gene.
There are many genetic contributors affecting things such as androgen signaling and follicular biology.
Different populations therefore inherited somewhat different distributions of susceptibility over many generations.
But genes aren't the whole story.
Researchers continue investigating environmental, metabolic and lifestyle factors that might influence onset or severity.
The important distinction is that those factors modify a biological process whose fundamental susceptibility is strongly genetic.
Did Civilization Cause Baldness?
Now we reach my favorite part of your question.
Perhaps modern people are balder because of:
stress,
pollution,
processed food,
sedentary life,
industrial chemicals,
or whatever else happened after somebody invented email.
There are certainly modern factors capable of affecting hair.
But there's one enormous problem with the idea that male-pattern baldness itself is a modern disease:
Ancient people were already bald.
Very bald.
Baldness Is Ancient
Concern about hair loss stretches back thousands of years.
The history of medical attempts to treat baldness reaches into ancient Egypt. A review in the British Journal of Dermatology traces hair-loss remedies to ancient Egyptian medical writings and notes treatments involving mixtures of substances that modern consumers would probably prefer not to massage into their scalps.
The historical medical literature describes concern about hair loss spanning roughly five thousand years.
So the hair-restoration industry didn't begin with late-night television.
Humanity has apparently been standing in front of reflective surfaces saying:
“Wasn't there more hair there last year?”
for an extremely long time.
Hippocrates Was Paying Attention
Around the fifth and fourth centuries BC, the Greek physician Hippocrates made an extraordinary observation.
He noted that eunuchs did not seem to become bald in the ordinary male pattern.
He obviously didn't know about:
testosterone
5-alpha-reductase
DHT
or
androgen receptors.
Those discoveries lay more than two millennia in the future.
But Hippocrates had noticed a connection between male reproductive biology and baldness.
That's remarkable observational medicine.
And it also proves beyond much reasonable doubt that recognizable male-pattern hair loss was already familiar in the ancient Mediterranean world.
Then Came the Remedies
Ancient hair-loss treatments could be... ambitious.
Egyptian and later Greco-Roman medical traditions experimented with various combinations of plant materials, animal fats and other substances in attempts to restore hair. Historical reviews even describe Hippocratic preparations involving ingredients that would make today's peptide serum seem downright conservative.
Apparently the Ibex Oil Salesman has a lineage.
A very, very long lineage.

Were Ancient People Just as Bald as We Are?
And here we have to disappoint everyone:
We don't know.
There was no ancient National Follicular Census.
Greek and Roman physicians left observations.
Artists depicted bald individuals.
Writers described them.
Statues preserve recognizable patterns of hair loss.
Ancient medical texts proposed treatments.
All of that proves baldness existed.
It does not give us a scientifically representative prevalence rate.
We can't responsibly say:
“Thirty-eight percent of Roman men aged 50 had androgenetic alopecia.”
The data simply don't exist.
But There Is a Huge Historical Complication: Life Expectancy
Suppose the underlying genetic susceptibility to androgenetic alopecia were exactly the same in ancient Rome as it is today.
Would the total population look equally bald?
Not necessarily.
Modern populations contain large numbers of people in their:
60s
70s
80s
and beyond.
And pattern hair loss becomes progressively more common with age.
Historical populations generally had very different age structures, particularly because mortality earlier in life was much higher.
Therefore, a modern population can contain more visibly bald people without modern people necessarily being genetically more prone to baldness.
That's a wonderfully counterintuitive possibility:
Perhaps modern civilization hasn't made us much better at going bald.
It has made us much better at surviving long enough to finish the job.

Does That Mean Baldness Rates Have Never Changed?
We can't say that either.
Populations migrate.
They mix.
Allele frequencies change.
Nutrition changes.
Disease burdens change.
Hormonal and metabolic health changes.
Environmental exposures change.
Medical treatments change.
And diagnostic awareness changes.
So it would be scientifically reckless to claim that androgenetic alopecia occurred at exactly the same frequency 2,000 years ago as it does today.
What we can say confidently is:
pattern baldness is ancient
and
there is no evidence that modern civilization invented it.
Could Stress Be Making Everyone Bald?
Not in the simple sense.
Stress and major physiological events can contribute to forms of shedding such as telogen effluvium.
That's different from androgenetic alopecia.
And we've already tackled the old idea that a “nervous disposition” explains ordinary baldness.
Male-pattern hair loss is fundamentally associated with genetic susceptibility and androgen-dependent follicular biology.
Modern stress can certainly affect hair.
It doesn't explain why Hippocrates was studying bald men 2,400 years ago.
So Who Is Most Likely to Go Bald?
If we're forced to summarize the current evidence:
Men of European ancestry appear among the populations most prone to androgenetic alopecia, particularly earlier and more extensive forms.
East Asian populations generally show lower prevalence and/or later onset, although pattern hair loss becomes increasingly common with age.
Some Black, Native American and Indigenous populations appear to have lower reported rates, but the comparative epidemiological evidence is less extensive.
Women throughout the world also experience pattern hair loss, with prevalence rising markedly with age and with substantial population differences.
But the single most reliable global observation may simply be:
The older the population, the more baldness you're going to find.
Baldness Doesn't Have a Country
And perhaps that's where our imaginary Baldness World Cup has to end.
There may indeed be populations with greater genetic susceptibility.
But baldness isn't:
European
American
Middle Eastern
Asian
or
African.
It's human.
It has crossed continents.
It has crossed cultures.
It has crossed social classes.
It has occupied royal palaces, monasteries, armies, farms, factories, boardrooms and probably every barber's chair ever constructed.
It was around before Rome.
It survived Rome.
It survived the Middle Ages.
It survived powdered wigs.
It survived the Industrial Revolution.
And it arrived intact in the twenty-first century, where we've surrounded it with genetics laboratories, pharmaceuticals, surgical transplantation, lasers, peptides, microneedles—and an astonishing assortment of shampoos promising that this time, finally, the Ibex has cracked it.

Humanity Didn't Invent Baldness. We Invented More Ways to Respond to It.
That's the real historical transformation.
An ancient man experiencing hair loss had limited options.
He could:
accept it
cover it
shave it
or
try whatever alarming concoction the local physician handed him.
Today someone experiencing hair loss can investigate:
- medical therapies;
- transplantation;
- scalp micropigmentation;
- cosmetic fibers;
- wigs;
- partial hair systems;
- full-cap systems;
- and increasingly sophisticated combinations of these approaches.
The biology is ancient.
Our ability to understand—and cosmetically manage—it is comparatively new.
So if you discover that your hairline is retreating, you haven't developed a strange modern affliction.
You've joined a fraternity old enough to include ancient Egyptians, Greeks and Romans.
And somewhere around 400 BC, Hippocrates was already looking at the same phenomenon and thinking:
“Now that's interesting...”
We've simply had another couple of thousand years to work on the answer.
